This clinical trial is investigating how the bacteria and other microorganisms in the mouth and gut of elderly patients might affect their risk of developing lung problems after elective upper abdominal surgery. Researchers are particularly interested in understanding if the types of bacteria present before surgery can help predict complications like pneumonia or low oxygen levels occurring within a week after the operation. The study involves collecting samples from participants before their surgery and analyzing them to see if there’s a link between these microorganisms and postoperative lung issues.

To be eligible for this study, participants need to be at least 65 years old and scheduled for a planned upper abdominal surgery that lasts more than two hours. They should be able to undergo general anesthesia and have a medium to high risk of developing pulmonary complications after surgery. Patients who have had recent surgeries, serious lung infections, or have used certain medications like antibiotics in the month before their surgery won't be included. Those who join the trial can expect to provide samples for testing and may help improve our understanding of how to prevent lung complications in elderly patients after surgery.

Eligibility criteria

  • Inclusion Criteria:
  • 1. Age: ≥ 65 years old;
  • 2. Surgery: Upper abdominal surgery (expected duration ≥ 2 hours);
  • 3. Anesthesia methods: general anesthesia, tracheal intubation;
  • 4. ASA classification: I-IV levels;
  • 5. Postoperative Pulmonary Complications Risk Score (ARISCAT): Medium to High Risk
  • 6. Patients or their families are able to understand the research protocol and are willing to participate in this study, providing written informed consent
  • Exclusion Criteria:
  • 1. Emergency surgery;
  • 2. This is the second surgery within the past month;
  • 3. Preoperative presence of pulmonary infection or other serious pulmonary complications
  • 4. Patients who have used antibiotics, probiotics, and acid suppressants within one month before surgery
