This clinical trial is studying how well ultrasound of the quadriceps muscle (the large muscle at the front of the thigh) compares to a tool called the Clinical Frailty Scale (CFS) for assessing frailty in older patients. Frailty is a condition that can make it harder for people to recover from illnesses or surgeries, especially in an intensive care setting. The goal is to see if using ultrasound can provide useful information about a patient's muscle health that might help in their treatment.

To be eligible for this trial, participants need to be at least 65 years old and admitted to the intensive care unit or outpatient department for less than a day. They also need to have had a recent abdominal CT scan or be scheduled for one soon after admission. If you or a loved one is considering participating, the study involves a quick ultrasound of the thigh muscle and completing the CFS, with no changes to usual care. Importantly, the patient's consent or that of a trusted family member is required before joining the study. This research aims to improve how we assess and support older patients in critical care.

Eligibility criteria

  • Inclusion Criteria:
  • patient aged 65 or over
  • admitted to intensive care or the outpatient department for less than 24 hours
  • with an abdominal CT scan less than 30 days prior to admission OR who are scheduled for an scheduled for a routine abdomino-pelvic CT scan within the first 24 hours of admission
  • Exclusion Criteria:
  • Neuromuscular disease
  • Lower limb amputation
  • Paraplegia \> 24 hours before admission
  • Clearly infected, burnt or lacerated ultrasound site.
  • Guardianship or curatorship
  • Opposition of the patient or his/her trusted support person or relative
  • Abdominal and pelvic CT scan not available in the previous month and not scheduled within the first 24 hours of treatment
