ClinConnect Summary

This clinical trial is studying a procedure called trabeculopuncture to see if it can help predict how successful another procedure, called ab interno trabeculectomy, will be in treating certain types of glaucoma, specifically open-angle glaucoma and pseudoexfoliation glaucoma. Trabeculopuncture is a test that checks how well fluid drains from the eye, which is important for managing eye pressure.

To participate in this trial, individuals must be at least 18 years old and have a type of glaucoma that isn’t well controlled with medication. They should also have signs of worsening eye health, like thinning of a specific part of the eye’s nerve layer or worsening vision. Participants can expect to undergo the trabeculopuncture test, which will help determine if they are good candidates for the trabeculectomy procedure. It's important to note that individuals with certain types of glaucoma, previous eye surgeries, or specific health conditions affecting eye pressure won't be eligible for this trial. This study is currently recruiting participants, so if you fit the criteria, there may be an opportunity to contribute to valuable research in glaucoma treatment.

Eligibility criteria

  • Inclusion Criteria:
  • individuals ≥ 18 years of age
  • clinical diagnosis of: primary open angle glaucoma, pigmentary glaucoma, or pseudoexfoliation glaucoma
  • uncontrolled intraocular pressure (IOP) with maximally tolerated medical therapy
  • progressive thinning of retinal nerve fiber layer assessed by Spectral domain optical coherence tomography (p \< 0.05)
  • progression of visual field defects
  • visually significant cataract and indication to reduce IOP or glaucoma medication burden
  • Exclusion Criteria:
  • clinical diagnosis of neovascular glaucoma, angle-closure glaucoma, uveitic glaucoma, neovascular glaucoma
  • prior glaucoma surgery
  • preexisting conditions affecting the episcleral venous pressure including Grave's ophthalmopathy, Sturge-Weber syndrome, Arteriovenous fistulas (carotid-cavernous fistula)
