This clinical trial is looking at how keeping blood flow in the external carotid artery affects the success of a specific surgery called carotid endarterectomy. This surgery is often done to treat narrowing of the carotid arteries, which can lead to strokes. The researchers want to compare the results of patients who have a graft placed (a piece of material used to bypass the blocked area) while maintaining blood flow in the external carotid artery versus those who do not.
To participate in this trial, individuals should be between the ages of 18 and 80 and have certain conditions related to carotid artery disease, such as significant narrowing in one or both carotid arteries. However, there are some exclusions, such as those who have had major surgery recently or have certain medical conditions like cancer or epilepsy. Participants can expect to receive careful monitoring throughout the study and will help researchers understand the best way to perform this surgery to improve patient outcomes. This research could ultimately help many patients at risk for strokes.

Eligibility criteria
- Inclusion Criteria:
- • symptomatic carotid stenosis (\> 50%)
- • unilateral asymptomatic carotid stenosis (\> 60%)
- • bilateral asymptomatic carotid stenosis (\> 60%)
- Exclusion Criteria:
- • carotid restenosis
- • "major surgery" in previous 6 months
- • previous brain trauma or surgery
- • malignant disease
- • epilepsy
- • carotid artery aneurysm
