This clinical trial is studying how certain measurements, like mechanical power and driving pressure, can help doctors predict how long patients with severe breathing problems will need to be on a breathing machine (called invasive mechanical ventilation). The goal is to find out which patients might be able to stop using the machine sooner, which ones should wait a bit longer, and for those who may need an extended time on the machine, to prepare for other options like a tracheostomy (a procedure to help with breathing).
To participate in this study, patients need to be 65 years or older and have recently been put on a breathing machine due to acute respiratory failure. Unfortunately, patients with lung cancer, those on a special type of breathing support called ECMO, or those who are expected to pass away within 24 hours are not eligible. If someone joins the study, they can expect to have their breathing measurements monitored closely to help guide their treatment decisions. This research is important because it could improve how doctors manage care for critically ill patients in the ICU.

- Inclusion Criteria:
- • consenting patients who have recently been intubated for acute respiratory failure
- Exclusion Criteria:
- • patients with lung cancer/ lung metastasis
- • ECMO
- • death within 24 hours
