Percutaneous Cryoablation of Intercostal Nerves for the Treatment of Rib Fractures

This clinical trial is studying a new treatment called percutaneous cryoablation to help manage pain and improve recovery for patients with multiple rib fractures. The researchers want to find out if this procedure not only reduces pain in the short and long term but also helps patients breathe better and enhances their overall quality of life. Participants will be compared to those receiving standard pain management to see how effective cryoablation is in improving their recovery.
To be eligible for the study, participants need to be over 18 years old, have more than two rib fractures, and have a pain score higher than 5 on a scale of 0 to 10. They should also have difficulty with breathing, as measured by a test called incentive spirometry. If eligible, participants will undergo the cryoablation procedure and will be monitored at different times afterward to assess their recovery. This trial is an important step in understanding how to better treat patients suffering from rib fractures and related complications.

Eligibility criteria
- Inclusion Criteria:
- • \> 18 years old
- • \> 2 rib fractures
- • Verbal Pain score \> 5
- • Incentive Spirometry \< 60% of predicted
- Exclusion Criteria:
- • Isolated Fractures of Ribs 1-3 and/or 10-12
- • Posterior Rib Fracture \< 4 cm from costovertebral joint
- • Sternal, Clavicle, Scapula Fracture
- • Thoracic Spinal Fracture of any type
- • Open Abdominothoracic Surgery
- • Unstable Spine
- • Extensive Subcutaneous Emphysema
- • BMI \> 35
- • Plan for Rib Plating
- • Chronic Opioid Use
- • Prior to study enrollment: Intubated, Traumatic Brain Injury, Dementia, Cognitive Impairment, Encephalopathy
- • Coagulopathy, Shock at time of ablation
- • Inability to participate in activities of daily living prior to injury
- • Home O2 use prior to trauma
- • Inhalation Injury
- • Rib Fractures due to cardiopulmonary resuscitation
- • Life Expectancy \< 6 months
- • \> 48 hours from injury
- • Pregnant, Incarcerated
