The DeliKet trial is studying whether a special way of feeding critically ill patients can help reduce the risk of postoperative delirium, which is a common and serious condition that can occur after surgery. The researchers want to see if giving patients a ketogenic diet, where they eat during the day and fast at night, is more effective than the usual method of continuous feeding around the clock. To participate in this study, you must be at least 18 years old and admitted to an intensive care unit (ICU) starting on enteral nutrition (a way of feeding through a tube). However, there are some health conditions that would exclude you from the trial, such as severe liver disease or certain metabolic disorders. If you qualify, you can expect to be part of a study that aims to improve recovery after surgery, and your health will be closely monitored throughout the process. This trial is not yet recruiting participants, but it aims to provide important insights into better nutrition strategies for critically ill patients.

Eligibility criteria
- Inclusion Criteria:
- • Age ≥ 18 years
- • Admission to an ICU
- • Start of enteral nutrition
- Exclusion Criteria:
- • Severe liver insufficiency / liver disease (Child Pugh \> B or 7-9 points)
- • Post total Pancreatectomy / Insulin-depending diabetes mellitus (IDDM)
- • Pregnancy / Lactation
- • Hemoglobin-concentration \< 80g/l
- • Severe metabolic disorder / severe autoimmune disease
- • Refractory respiratory or metabolic acidosis
- • Disorder of mitochondrial transportation of fatty acids
- • Disorder of the oxidation of fatty acids
- • Disorder of gluconeogenesis, der ketone body production or ketone body degradation
- • Intermittent porphyria
- • Severe arrhythmia / cardiomyopathy
- • Contraindications against enteral nutrition
- • Missing informed consent
