This clinical trial is focused on helping older Alaska Native and American Indian adults with serious illnesses have important conversations about their medical care preferences, known as advance care planning. These conversations help make sure that the care they receive matches their values and wishes. The study is testing a new program called JUMPSTART-ANAI, which is designed specifically for this community to encourage these talks between patients and their primary care doctors. Researchers want to see if this program works better than the usual care and to learn how to best use it in healthcare settings that serve Alaska Native and American Indian adults.
People who might join the study are Alaska Native or American Indian adults aged 40 or older who have a serious illness like cancer, lung disease, or kidney problems. They need to be seeing a primary care provider through Southcentral Foundation and have visited their doctor at least twice in the past year. Participants should not have an advance directive on file, which is a legal document stating their healthcare wishes. Those with significant memory problems or who can’t complete a 30-minute visit by phone or in person won’t be eligible. If you join, you can expect to take part in discussions about your healthcare preferences with your doctor, supported by the new program, helping ensure your care reflects what matters most to you.

Eligibility criteria
- Inclusion Criteria:
- • Alaska Native and/or American Indian
- • Age 40 or older
- • Empaneled to a Southcentral Foundation primary care provider
- • Diagnosed with at least 1 serious illness (e.g., cancer, COPD, kidney disease)
- • Has made 2 or more to primary care in last 12 months
- • No Advance Directive documented in electronic health record
- Exclusion Criteria:
- • Cognitive impairment (≥ 2 errors) as assessed by the validated Short Portable Mental Status Questionnaire
- • Not able to complete 30-minute study visit by phone or in person
