This clinical trial is investigating a personalized self-care support program designed for patients with diabetic foot ulcers (DFUs). The goal is to see if this program, which focuses on helping patients manage their own care through empowerment and active listening, is acceptable and easy to use for both patients and wound care nurses. Researchers want to find out if this program can effectively support self-care for patients receiving treatment in primary care settings.
To be eligible for this study, participants must be at least 21 years old, have diabetes, and be receiving treatment for their foot ulcers at National Healthcare Group Polyclinics. During the trial, participants can expect to attend three 30-minute face-to-face sessions spaced out over a few weeks after their regular wound care appointments. The study is currently recruiting participants, and it’s important to note that individuals with certain serious health conditions or who are pregnant will not be included in the trial.

Eligibility criteria
- Inclusion Criteria:
- • Patients with diabetes and DFU aged 21 years or above receiving treatment and wound care at National Healthcare Group Polyclinics
- • HCPs, who are aged 21 years or above with a patient-facing role working in delivering program (only for qualitative interviews)
- Exclusion Criteria:
- • Patients have a diagnosis of critical lower limb ischemia, active osteomyelitis, Charcot foot, cognitive/psychiatric diagnoses, and hearing or vision impairment.
- • Pregnant women will be excluded from the study for both patient and HCP groups
