This clinical trial is looking at a new type of treatment called Cognitive Functional Therapy (CFT) for people who have had shoulder pain for a long time, known as chronic shoulder pain. The study will compare CFT to traditional therapeutic exercises to see which method is more effective. Researchers want to understand how easy it is for people to join the study, how well the assessments work, and if participants are comfortable with being assigned to different treatment groups. They will also look at how many people stay in the study over time and if the results match what other studies have found.
To be eligible for this trial, you need to be between 18 and 60 years old and have had shoulder pain for over three months, with a pain rating of at least 4 on a scale of 0 to 10. However, certain conditions like past shoulder surgeries, specific shoulder injuries, or other serious health issues may prevent participation. If you join the study, you will receive either CFT or therapeutic exercises and will be asked to attend assessments to track your progress. Eligibility criteria
- Inclusion Criteria:
- • Men and women;
- • Age between 18 and 60 years;
- • Presence of shoulder pain for more than 3 months;
- • Pain intensity of 4 points or more on the 11-point Numeric Pain Rating Scale during the past week.
- • High level of disability or moderate level of irritability.
- Exclusion Criteria:
- • History of fracture or surgery of the clavicle, scapula, and/or humerus, surgical stabilization or rotator cuff repair;
- • History of dislocation, instability (positive sulcus sign or apprehension test), and/or rotator cuff tear (positive drop arm test);
- • Adhesive capsulitis verified by the presence of gradual onset pain associated with stiffness and reduced passive and active mobility;
- • Ongoing pregnancy;
- • Reproduction of shoulder pain radiating to the entire upper limb during cervical or thoracic spine tests (positive Spurling test);
- • Neurological or systemic diseases that may alter muscle strength and sensitivity such as rheumatoid arthritis, fibromyalgia, lupus, gout, and diabetes;
- • Corticosteroid injection in the shoulder region in the last 3 months;
- • Physiotherapy treatment in the last 6 months.
