ClinConnect Summary

This clinical trial is studying small nerve fiber activity in patients with Carpal Tunnel Syndrome (CTS) using a method called Quantitative Sensory Testing (QST). While traditional tests can check large nerve fibers, this study focuses on smaller nerve fibers that may also be affected in CTS. Researchers want to see how going through standard physical therapy might change these small nerve fibers and whether these changes could impact how patients feel and function.

To be eligible for this study, participants need to be between 18 and 65 years old, have pain or tingling in the part of the hand supplied by the median nerve, and show positive results on specific tests called the Phalen and Tinel tests. Participants will need to attend physical therapy sessions, and those with certain other nerve issues, recent surgeries, or trauma won't be able to join. If you decide to participate, you can expect to go through physical therapy while researchers monitor your nerve activity and how it relates to your symptoms. This study is currently recruiting, and both men and women are welcome to join.

Eligibility criteria

  • Inclusion Criteria:
  • DEERS eligible Age 18-65 Pain and paresthesia in the median nerve distribution A positive Phalen test A positive Tinel test over the carpal tunnel Willing to attend physical therapy
  • Exclusion Criteria:
  • Nerve root signs consistent with a radiculopathy Suspected radial and ulnar nerve involvement based on monofilament testing Bilateral CTS Previous hand surgery Injections in the upper quarter in the last 6 months Cervical, shoulder, or UE trauma in the last 6 months Currently pregnant or postpartum within the last 6 months No more than minimal care (evaluation and patient education, 1 visit) for the current episode
