This clinical trial, called Primary Care-GI Connect, is exploring ways to improve the number of people who get follow-up colonoscopies after receiving an abnormal result from a stool test designed to check for colorectal cancer. Colorectal cancer screenings are important because they can help catch cancer early when it is more treatable. However, many people do not follow up with the necessary colonoscopy after an abnormal stool test, especially in community health centers that serve low-income patients. The Primary Care-GI Connect program aims to address this issue by improving how care is coordinated between doctors and making the referral process for colonoscopies smoother.
To participate in this trial, you need to be an adult patient at one of the six clinic sites of the NEVHC (Northeast Valley Health Corporation). The trial is not yet open for recruiting participants, but it will involve working closely with both your primary care doctor and specialists to ensure you receive the care you need after an abnormal test result. This initiative aims to make sure that more patients follow through with their colonoscopy, ultimately helping to reduce the risk of colorectal cancer in the community.
