This clinical trial, called the tGLB-CVA study, is looking at a new way to help people who have had a stroke improve their health through a program delivered online. The program focuses on lifestyle changes, like diet and exercise, and will be tested against a group that will have to wait for six months before starting. The goal is to see if this telehealth approach can help people who are at least 12 months post-stroke lead healthier lives.
To participate in this study, you need to be between 65 and 74 years old, have had any type of stroke, and have a body mass index (BMI) of 25 or higher. You should also be able to engage in physical activity and have access to the internet or a phone. However, individuals with diabetes, certain medical conditions affecting physical activity, low cognitive function, or who are pregnant cannot join. If you decide to participate, you will receive support from a team that includes patient partners and peer mentors, making it a collaborative and encouraging experience.

Eligibility criteria
- Inclusion Criteria:
- • Body Mass Index ≥25
- • All types of stroke
- • Able to Participate in Physical Activity
- • ≥12 months post first stroke
- • Have internet, phone, or computer access, or be willing to use one provided by the study team
- Exclusion Criteria:
- • Contraindications for physical activity
- • Low Cognitive Function
- • Residing in hospital, acute rehab, skilled nursing facility
- • Not fluent in the English language
- • Pre-existing eating disorder
- • Pregnancy
