This clinical trial is studying ways to improve how people with diabetes stick to their medication routines. Researchers want to find the best methods to encourage patients to meet with their clinical pharmacists, who can help them manage their diabetes better. The trial will take place in community settings and will involve patients who are either struggling to manage their diabetes or not taking their prescribed medications as directed. To be eligible, participants need to be at least 18 years old, speak English or Spanish, and be receiving care from a specific primary care provider.
Participants in this study can expect to be randomly assigned to different groups, each using different methods to encourage them to meet with a clinical pharmacist. Some groups may receive mailed information, while others may get different types of letters or varying numbers of phone calls. The main goal is for participants to schedule and complete an appointment with a pharmacist within eight weeks. During this time, researchers will also look at how well participants are following their medication plans and how their diabetes control improves over three months. This study aims to find effective ways to help patients manage their diabetes and improve their health outcomes.

Eligibility criteria
- Inclusion Criteria:
- • ≥18 years of age
- • English or Spanish speaking
- • Receiving care from a BMC primary care provider
- • Non-adherent to prescribed oral glucose-lowering medications as per pharmacy dispense records (proportion of days covered \<80% to at least one eligible medication in last 6 months)
- • Evidence of poor or worsening disease control
- Exclusion Criteria:
- • Evidence of terminal conditions
