This clinical trial is studying the effects of a supplement called myo-inositol during pregnancy to see if it can help prevent gestational diabetes, a type of diabetes that can develop during pregnancy. Myo-inositol is a natural substance that can improve how the body uses insulin, which is important for keeping blood sugar levels stable. The trial will involve giving eligible women either myo-inositol or a placebo (a non-active substance) to compare their effects on blood sugar control.
To be eligible for the trial, participants need to be women aged between 18 and 45 who are pregnant with one baby and are considered obese before pregnancy (with a body mass index of 30 or higher). They should be receiving prenatal care at Tufts Medical Center and plan to give birth there. Participants will be required to wear a continuous glucose monitor (CGM) to track their blood sugar levels and will be asked to sign consent to join the study. This trial is not yet recruiting participants, but it aims to provide valuable information on how myo-inositol may help improve health outcomes for pregnant women at risk of gestational diabetes.

Eligibility criteria
- Inclusion Criteria:
- • Singleton gestations
- • Women aged \> 18 years and \< 45 years
- • Recruited before 16 weeks gestation
- • Obese (pre-pregnancy BMI ≥ 30)
- • Receiving prenatal care at Tufts Medical Center
- • Planning to give birth at Tufts Medical Center
- • Can tolerate glucose tolerance test
- • Willing and able to wear CGM
- • Willing and able to sign informed consent
- Exclusion Criteria:
- • Multiple gestation
- • Preexisting diabetes
- • Taking medications that impact body weight or metabolism (eg metformin)
- • Inability to tolerate glucose tolerance test
- • Adults unable to consent (cognitively impaired adults)
- • Wards of the state
- • Non-viable neonates
- • Neonates of uncertain viability
