Defining the Risk of Ventricular Tachycardia in Genetic Cardiomyopathies

This clinical trial is studying how certain tests can help identify patients with genetic heart conditions who may be at higher risk for dangerous heart rhythms, specifically ventricular tachycardia. Researchers want to see if using advanced imaging techniques like cardiac MRI and special mapping during heart procedures can help find out more about the heart's electrical activity and any damage that could lead to these serious issues.

To participate in this study, you need to be an adult aged 18 or older, have a history of atrial fibrillation (an irregular heartbeat), frequent premature ventricular contractions (extra heartbeats), or ventricular tachycardia diagnosed before age 60. You should also be scheduled for a specific heart procedure to help treat these conditions. Participants will have a cardiac MRI before their procedure and undergo voltage mapping during it. This trial is open to all genders and is currently recruiting participants. Eligibility criteria

  • Inclusion Criteria:
  • Adults aged 18 and older
  • Diagnosed with AF, frequent PVCs, or VT before age 60
  • Scheduled for catheter-based AF ablation (de-novo or repeat) OR catheter-based PVC ablation OR catheter-based VT ablation
  • Able to provide written, informed consent
  • P/LP variant in TTN or other CM gene (cases) or identified as a genotype-negative control.
  • Exclusion Criteria:
  • Diagnosed with a genetic CM or arrhythmia syndrome prior to ablation procedure
  • VUS in 'possibly pathogenic' subgroup (control group only)
  • Previous PVC or VT ablation
  • LVEF \<20%
  • Prosthetic mitral or aortic valve
  • Contraindication to heparin
  • Prior myocardial infarction
