This clinical trial is studying how personalized follow-up care from a sport-health professional can help patients with chronic respiratory diseases maintain their improvements after a pulmonary rehabilitation program. Pulmonary rehabilitation is a treatment that helps individuals with breathing problems improve their physical abilities, quality of life, and symptoms. The researchers want to see if having tailored support after this program can provide long-term benefits compared to just receiving standard care.
To be eligible for this study, participants should be diagnosed with one or more chronic respiratory diseases, such as asthma or chronic obstructive pulmonary disease (COPD), and must have completed a pulmonary rehabilitation program. Participants will be divided into two groups: one will receive personalized follow-up support, while the other will get usual care. The trial is open to adults aged 65 to 80, and all genders are welcome. If you decide to join, you can expect regular check-ins and support to help you stay on track with your health goals after the rehabilitation program.

Eligibility criteria
- Inclusion Criteria:
- • Diagnosis of one or more chronic respiratory diseases (asthma, chronic obstructive pulmonary disease, diffuse interstitial lung disease, bronchiectasis, obstructive sleep apnea syndrome, etc.).
- • Participation in a respiratory pulmonary program
- • Have given consent
- • Patient of legal age
- • Patient affiliated to Social Security
- Exclusion Criteria:
- • Any medical contraindication to the practice of adapted physical activity in rehabilitation
- • Patient refusal
- • Patients under guardianship or curatorship
- • Patients deprived of their liberty
- • Minors
- • Patients over 80 years of age at the time of inclusion.
