This clinical trial is studying the effects of aerobic and strength exercises on patients with temporomandibular disorders (TMD) who experience chronic pain, known as nociplastic pain. This type of pain can cause increased sensitivity to touch and pressure, fatigue, and difficulty concentrating. The researchers want to find out if adding these exercises to regular physical therapy can help improve pain and other symptoms more effectively than physical therapy alone.
To be eligible for this study, participants must be diagnosed with a specific type of TMD by a dentist and have experienced chronic pain for more than three months. They should also be physically able to perform aerobic and strength exercises. However, individuals with certain medical histories, such as recent injuries or surgeries, systemic diseases, or those currently undergoing orthodontic treatment, will not be able to participate. If chosen for the trial, participants can expect to engage in a structured exercise program while receiving regular physical therapy to help manage their pain.

Eligibility criteria
- Inclusion Criteria:
- • Diagnosis of a myogenic temporomandibular disorder (DC/TMD) by an odontologist.
- • Chronic pain (\>3months)
- • Nociplastic pain
- • Positive Flexion Rotation Test
- • Be physically able to realize aerobic and strength exercise (functional movement test)
- Exclusion Criteria:
- • History of trauma, TMJ or cervical fracture the past three months
- • History of TMJ or cervical surgery
- • Systemic, rheumatic, metabolic, neurologic, psychiatric, pulmonary diseases or neoplastic malignant
- • History of cardiovascular diseases which contraindicate aerobic exercise
- • Current orthodontic treatment, splints for bruxism
- • Drug addiction, alcoholism
- • Pregnancy
- • Use of analgesic medication less than 48 hours before each data collection
- • Physical therapy treatment during the past three month
