This clinical trial, titled "Remote Assessment of Signs and Symptoms in Palliative Care," is exploring how well remote monitoring can help patients receiving palliative care. Palliative care focuses on providing relief from the symptoms and stress of serious illnesses. The study aims to find out if using technology to check on patients’ health from home can improve their experience and outcomes. Participants will fill out questionnaires regularly about how they feel and will use equipment to measure their vital signs, like heart rate and temperature, every day. This information will be shared with their healthcare team, allowing them to address any issues quickly. After a month, both patients and their caregivers will share their thoughts on how helpful this remote monitoring has been.

To participate in this trial, individuals need to be aged 65 and older, have advanced cancer or another serious illness, and have a prognosis (expected time left to live) of more than three months. They should also be receiving care at home and have a family member helping them. Patients who are not able to provide consent due to cognitive issues, those living in care facilities, or who speak a language other than English are not eligible for this study. This trial is currently recruiting participants, and it offers an opportunity to potentially improve care for patients and their families during challenging times.

Eligibility criteria

  • Inclusion Criteria:
  • Primary diagnosis of advanced cancer or another life-limiting condition
  • Estimated prognosis greater than 3 months
  • Referred for community-based specialist palliative care
  • Living in own home (or home of family carer)
  • Living with a family carer
  • Exclusion Criteria:
  • Estimated prognosis less than 3 months or more than 12 months
  • Cognitive impairment - unable to provide consent / complete assessments
  • Non-English speaker
  • Living in care home / nursing home / hospice / community hospital
  • Not living with a family carer
