This clinical trial is designed to study patients with acute heart failure, a condition where the heart struggles to pump blood effectively, leading to symptoms like shortness of breath and swelling. The main goal of the study is to identify different types of congestion in these patients and see how these types affect their treatment outcomes. The researchers will look at how well patients respond to therapy, their chances of being readmitted to the hospital or passing away within six months, the length of their hospital stay, and changes in heart pressure over time. Additionally, they will explore the relationship between congestion and liver health.
To participate in this trial, individuals must be at least 18 years old and diagnosed with acute heart failure requiring specific treatment. Participants will undergo various tests, including ultrasound exams of the heart, lungs, and kidneys, and a special scan of the liver to assess its health. If you or a loved one are considering joining this study, you will be helping researchers understand more about heart failure and its treatment, but you must be willing to give consent and be able to follow study instructions.

Eligibility criteria
- Inclusion Criteria:
- • Able and willing to give written informed consent
- • Age ≥ 18 years
- • Male or female
- • Primary diagnosis of acute heart failure at presentation, with signs and symptoms of tissue decongestion. Diagnosis is based on the criteria in the ESC HF guidelines (1)
- • Requirement of intravenous loop diuretics
- Exclusion Criteria:
- • Patients with severe kidney dysfunction (in need for ultrafiltration or dialysis)
- • Previous participation in this study
- • Inability to follow instructions
- • Dyspnoea or oedema primarily due to non-cardiac causes
