This clinical trial is designed to compare two different surgical approaches for treating hemorrhoids, which are swollen veins in the lower rectum or anus that can cause discomfort and pain. Specifically, the study will look at the differences in pain levels and complications between a standard hemorrhoid surgery (called hemorrhoidectomy) and a surgery that includes an additional step called lateral internal sphincterotomy, which involves cutting a small muscle to help relieve pressure.
To participate in this trial, individuals need to be between 18 and 60 years old and have been diagnosed with moderate to severe hemorrhoids (Grade II, III, or IV). Unfortunately, those under 18, with complete rectal prolapse, or with mild hemorrhoids (Grade I) will not be eligible. If selected for the trial, participants can expect to undergo one of the two surgical procedures and will be monitored for pain and any complications afterward. This research aims to help doctors better understand which approach may be more effective and safer for patients dealing with hemorrhoids.

Eligibility criteria
- Inclusion Criteria:
- • 1. Surgically fit according to ASA.
- • 2. Cases with hemorrhoids Grade ll\& III \& IV.
- • 3. Age between (18-60).
- Exclusion Criteria:
- • 1. Age less than 18 years old.
- • 2. Complete rectal prolapse.
- • 3. Grade I hemorrhoids.
