This clinical trial, called the Mobile Intervention for Suicidal Thoughts (MIST), is studying a new program designed to help reduce suicidal thoughts in veterans. Researchers believe that certain negative beliefs about oneself and the world can increase the risk of suicide. MIST aims to change these harmful beliefs using a mobile app, making it easier for veterans to access support wherever they are. The goal of this pilot trial is to see if using this app is a practical and helpful addition to the care veterans already receive for their mental health.
To participate in this study, veterans must be receiving care at the Durham VA Health Care System and have experienced thoughts of suicide in the past month without planning to act on them. They should also be able to read basic materials and meet regularly with a mental health provider. However, those currently facing severe mental health issues, such as active psychosis or severe substance use disorders, will not be eligible for this trial. If chosen to participate, veterans can expect to use the MIST app alongside their usual treatment and provide feedback on their experience with it.

Eligibility criteria
- Inclusion Criteria:
- • Veterans using the Durham VA Health Care System
- • Endorsing suicidal ideation without intent in the last 30 days, established via C-SSRS
- • Meet at least every 4 months with a mental health provider, established via chart review chart
- • Can read at least 6th grade level material, established by their demonstrated ability to read the consent form and answer basic questions about it
- Exclusion Criteria:
- • Currently in a period of active psychosis or mania, established via the SCID-5
- • Currently meeting criteria for a moderate or severe alcohol or substance use disorder, established via the SCID-5
- • Endorsing suicidal intent, established via C-SSRS
- • Exhibit current prominent suicidal or homicidal ideation requiring immediate intervention
- • Have used the MIST intervention before as part of another research study
