The ACHIEVE P1-EPI trial is part of a larger project aimed at improving heart health and reducing health inequalities in Detroit, Michigan, and Cleveland, Ohio. This study focuses on understanding and addressing high blood pressure, also known as hypertension, in patients who visit the Wayne Health Mobile Health Unit. By looking at factors that affect blood pressure and health outcomes, the researchers hope to identify ways to help people manage their blood pressure better and improve overall health.
To participate in this trial, you need to be at least 18 years old and have high blood pressure (which means a systolic blood pressure of 120 or higher and/or a diastolic blood pressure of 80 or higher). You should also have a phone that can receive text messages and be willing to allow follow-up check-ins about your health. Unfortunately, pregnant women, children under 18, and those who are currently involved in other trials cannot participate. If you join, you can expect to share information about your health and receive support in managing your blood pressure over time. This is an important step in finding better ways to help people lead healthier lives.

Eligibility criteria
- Inclusion Criteria:
- • Mobile Health Unit Patients with blood pressure above normal (≥120 systolic and/or ≥80 mmHg)
- • Has a phone with the ability to receive text messages
- • 18+ years old
- • Consent to allow prospective follow-up including through EHR review
- Exclusion Criteria:
- • Non-mobile health unit patients
- • MHU patients with systolic BP \< 120 mmHg AND diastolic BP \< 80 mmHg
- • Pregnant Women
- • Children less than 18 years old
- • Individuals viewed by the investigative team as unable to understand and sign the informed consent form
- • Currently enrolled in another on-going interventional trial initiated on the mobile health unit
