This clinical trial is looking at a new way to help patients with moderate to severe Acute Respiratory Distress Syndrome (ARDS), a serious condition that makes it hard to breathe due to lung damage. Patients with ARDS often need mechanical ventilation, which helps them breathe but can also harm the lungs. The study aims to test a special technique called selective lung recruitment, which involves using a device to open up specific parts of the lung to improve oxygen levels while reducing the risk of injury. By doing this, the researchers hope to find a safer way to enhance oxygenation for patients.
To participate in this study, patients must be between the ages of 65 and 74, have ARDS with a certain level of oxygenation difficulty, and be under sedation. They should also have specific lung issues visible on imaging tests. However, those who are pregnant, have unstable heart conditions, or are under guardianship won't be eligible. Participants will undergo this innovative lung recruitment technique, and researchers will use a CT scan to check how well the lungs expand afterward. This trial is still in the planning stages, so no participants have been recruited yet.

Eligibility criteria
- Inclusion Criteria:
- • acute ARDS according Berlin definition and a PaO2/FiO2 ratio inferior to 200
- • patient under sedation and muscular blockade
- • postero-basal consolidation on CT or pulmonary ultrasound
- • affiliated to the French social insurance system
- Exclusion Criteria:
- • hemodynamic instability
- • pregnancy
- • patient under guardianship
