ClinConnect Summary

This clinical trial is looking at how a specialized clinic, called the PRIME clinic, can help older adults who are eligible for open-heart surgery. The study wants to find out if meeting with the PRIME clinic team affects the treatment choices doctors recommend, improves the patients' quality of life, and influences healthcare costs. In simple terms, they want to see if personalized care and discussions about treatment options can help patients feel better and make better use of healthcare services.

To participate in this study, you need to be 70 years or older, be eligible for heart surgery, and have at least two health risk factors, such as a history of stroke, breathing problems, or reduced mobility. If you join, you’ll visit the PRIME clinic once, fill out surveys about your quality of life at two points during the study, and track your healthcare visits over a year with additional questionnaires. This study is currently recruiting participants, and it is open to everyone, regardless of gender, as long as you can read and understand Dutch.
