This clinical trial is focused on improving the care of young children with bronchiolitis, which is a common lung infection that often leads to hospital stays. The study aims to test a new way to help babies breathe better by using a device called a high-flow nasal cannula (HFNC), which delivers oxygen through a tube in the nose. The goal is to see if having a structured plan for starting and stopping the use of HFNC can help reduce its use and improve patient outcomes in the Pediatric Intensive Care Unit (PICU) at Aydın Maternity and Children's Hospital.
To participate in this trial, children must be between 1 and 24 months old and need HFNC treatment for their breathing issues. They should also have moderate to severe respiratory problems but should not have any serious heart or lung conditions that could complicate their care. Parents will need to provide written consent for their child's participation after being fully informed about the study. During the trial, families can expect close monitoring and support from healthcare professionals as they work to manage their child’s condition effectively.

Eligibility criteria
- Inclusion Criteria:
- • Patients older than 1 month and younger than 24 months of age; hospitalized at the PICU with the intention of treatment with HFNC at least for the upcoming 4 hours
- • Patients with moderate and severe RAS(Respiratory Assessment Scale) requiring HFNC
- • Written informed consent signed and dated by the patient or one relative in case that the patient is unable to consent, after full explanation of the study by the investigator and prior to study participation
- Exclusion Criteria:
- • İnfants born before 32 weeks of gestation or those with cardiopulmonary, genetic, congenital, or neuromuscular abnormalities.
- • Patient with an indication for immediate noninvasive ventilation (NIV)
- • Patients deemed at high risk for the need for non-invasive mechanical ventilation within the next 4 hours
- • Hemodynamic instability
