The PREPARE-HOME trial is studying whether a special program called prehabilitation can help frail elderly patients improve their physical health before having major surgery. This program uses smart wearable technology, like a Fitbit, to track important health information and guide patients through a personalized exercise routine. The goal is to see if this approach can help them walk better before surgery and recover more smoothly afterward compared to those who only receive standard care.

To be eligible for this trial, participants need to be 65 years or older, scheduled for a major non-heart-related surgery, and considered frail based on specific health assessments. They should also have a smartphone to use with the wearable device and be available for at least two weeks before their surgery to engage in the prehabilitation program. Throughout the trial, participants will be monitored remotely and will complete tests to evaluate their physical ability and overall quality of life. This study is currently not recruiting participants but aims to provide valuable insights into improving recovery for elderly patients facing surgery.

Eligibility criteria

  • Inclusion Criteria:
  • Frail elderly patients aged 65 years or older.
  • Scheduled to undergo major elective, non-cardiac surgery.
  • Anticipated surgery duration of over two hours or expected blood loss greater than 500 mL.
  • Frailty status with Edmonton Frail Scale (EFS) score of 6 or higher.
  • Capacity to provide informed consent independently.
  • Available at least two weeks before surgery to participate in the prehabilitation program.
  • Ability to follow exercise instructions independently.
  • Ownership of a smartphone with Bluetooth and cellular connectivity.
  • Exclusion Criteria:
  • Severe comorbidities that interfere with participation in the exercise program (e.g., unstable angina, severe aortic stenosis, or uncontrolled arrhythmias).
  • Cognitive impairments that prevent understanding or adherence to the intervention.
