This clinical trial is called "Sexual and Urinary Function Improvement for Cancer Survivors." It aims to help improve sexual and urinary problems that may affect men who have survived prostate cancer. The study will take place at NYC Health + Hospitals/Bellevue, which is the oldest public hospital in the United States. Researchers want to see if a special care model, which includes a team of trained professionals like care managers and nurse practitioners, can effectively support patients in managing these issues.
To join the study, participants need to be at least 18 years old and have been diagnosed with prostate cancer. They should also have sought treatment for their condition. Unfortunately, those under 18, or individuals who have not received care for prostate cancer, won't be eligible to participate. If someone decides to join, they can expect to work closely with a dedicated team that will help them address their sexual and urinary concerns in a supportive environment. This trial is currently not recruiting participants yet, but it represents an important step in understanding and improving the quality of life for prostate cancer survivors.

- Inclusion Criteria:
- • At least 18 years old
- • Have a diagnosis of prostate cancer
- • Have sought care/treatment for prostate cancer
- Exclusion Criteria:
- • Patients under 18 years old
- • Patients who have not sought care/treatment for prostate cancer
- • Patients who are categorized as "vulnerable subjects," such as minors or incarcerated individuals.
