This clinical trial is studying a new approach called active surveillance for patients with certain thyroid nodules that have uncertain or suspicious findings (known as Bethesda categories IV-VI). Instead of immediately performing surgery, which can lead to complications, the study aims to monitor these nodules over time with regular ultrasounds and checkups. The goal is to see if this strategy can safely delay or even eliminate the need for surgery in some patients, especially in Colombia, where data on this approach is limited.
To participate in the trial, individuals must be over 18 and have been diagnosed with Bethesda IV-VI thyroid nodules that are low-risk, meaning they haven't spread and don’t show signs of severe cancer. Patients who have had previous head and neck cancers or signs of advanced cancer are not eligible. Throughout the study, participants will have their nodules monitored for any changes, helping researchers understand how effective active surveillance can be in avoiding unnecessary surgeries and improving patient care.

Eligibility criteria
- Inclusion Criteria:
- • 1. Patients with thyroid nodules and FNAC with a report corresponding to Bethesda category IV-VI, treated at the following health centers: HUSVF, HAMA, Center of Excellence in Head and Neck Diseases - CEXCA, Somer Clinic.
- • 2. Patients classified as low-risk tumors (intrathyroidal, with no evidence of locoregional or distant metastasis).
- • 3. Patients not subjected to thyroidectomy as the initial treatment.
- Exclusion Criteria:
- • 1. Patients with a second head and neck cancer. 2. Patients with clinical or imaging evidence of thyroid malignancy with lymph node or distant metastasis confirmed by pathology.
