This clinical trial is studying whether giving patients two doses of a medication called dexamethasone, along with another medication called tranexamic acid, can help improve recovery after hip or knee replacement surgeries. The researchers want to see if this combination can reduce inflammation (measured by C-reactive protein), decrease pain and fatigue, improve how well patients can move their joints, and shorten their hospital stay after surgery.
To participate in the trial, patients must be at least 45 years old, scheduled for either a hip or knee replacement due to severe arthritis, and have a pre-operative hemoglobin level above 10g/dL. Participants will answer some questions before surgery and again on the first day after surgery, as well as one week and four weeks after they leave the hospital. This study is currently looking for volunteers, and both men and women are welcome to join. Overall, the trial aims to find out if this treatment approach can make recovery from joint replacement surgery easier and faster for patients.

Eligibility criteria
- Inclusion Criteria:
- • Patients scheduled for unilateral or bilateral total joint replacement surgery in grade 4 osteoarthritis.
- • Patients having pre-operative hemoglobin above 10g/dL.
- • Include both genders.
- • Patients having age more than 45 years.
- • Exclusion Criteria
- • Patients having history of thromboembolic event.
- • Patients having tranexamic and steroidal allergy.
- • Patients having pre operative Hemoglobin level less than 10g/dL
- • Patients having history of clotting disorders including abnormal prothrombin time, prothrombin time test, or international normalized ratio.
