This is an observational study being conducted at Brest University Hospital in France to see if a Sepsis Performance Improvement Program helps emergency teams treat sepsis and septic shock more quickly and in line with 2021 guidelines. Researchers will compare care from before the program (2017) to care after the program started (2022–2024) to see if guideline use and patient outcomes—like need for ICU care, length of hospital stay, and risk of death—improve. The study is currently enrolling and no results are available yet.
The study includes adults 18 and older who come to the emergency department with sepsis or septic shock and have positive blood cultures and/or a positive SOFA score (a measure of organ problems), as confirmed by the hospital’s sepsis team. About 600 people are expected to be studied. Participation involves using information from routine care (no extra tests or treatments beyond standard care), with data collected from medical records. Eligible patients are invited to participate, and the study is expected to finish around mid-2026. The researchers will look at outcomes such as time to first antibiotic, quality of fluid and oxygen therapy, progression to septic shock, ICU and hospital stay length, duration of ventilation, changes in organ function scores, and death within 28 or 90 days.

- Inclusion Criteria:

- • Patients with positive blood cultures and a positive SOFA score
- • Patients with blood cultures collected, a positive SOFA score, and sepsis/septic shock confirmed by the sepsis team at Brest University Hospital.
- • Patients enrolled in a social security scheme
- Exclusion Criteria:
- • Patients under judicial protection (guardianship, curatorship, etc.)
- • Refusal to participate
- • Pregnant patients
