This is a randomized, triple‑blind interventional trial at Mansoura University in Egypt. Forty adults who need to have a non-restorable lower jaw molar tooth extracted and replaced with an immediate dental implant will be randomly assigned to one of two treatment groups. Both groups receive an immediate implant with bone support and a membrane, but one uses a dentin-derived membrane made from the extracted tooth, and the other uses an extended platelet-rich fibrin membrane. The goal is to compare how well each membrane helps keep the implant stable over the first year after placement. Implant stability will be measured with the Osstell device at the time of placement and then at 3, 6, and 12 months.
To be eligible, adults ages 18 to 60 must have a single lower molar that needs extraction and immediate implant placement with guided bone regeneration, enough space for a future prosthesis, and good overall health without conditions that would make implant surgery risky. They should be cooperative with good oral hygiene and not have bruxism (teeth grinding) or be smokers or pregnant. Exclusions include any local or systemic issues that could interfere with the procedure or recent head/neck radiation. The study started in mid-2024 and is currently enrolling by invitation, with an estimated completion in 2025. If eligible and interested, you would be treated at the Oral and Maxillofacial Surgery Department of Mansoura University.

Eligibility criteria
- Inclusion Criteria:
- • Age above 18 years
- • Non-restorable mandibular molar tooth that needs dental extraction \& guided bone regeneration for immediate implant placement.
- • Acceptable inter-arch space for the future prosthesis.
- • Patient medically free from systemic diseases that absolutely contraindicate implant surgery.
- • Cooperative patient with high motivation and acceptable oral hygiene.
- • Free from history of bruxism / parafunctional habits.
- Exclusion Criteria:
- • Local and / or systemic conditions that interfere with the procedure.
- • Smokers.
- • Pregnancy.
- • Uncooperative patients.
- • Patient had undergone in the past 12 months radiation therapy to the head and neck area.
