This clinical trial is looking at a new way to manage pain during hand, wrist, forearm, and elbow surgeries by using a technique called truncal blocks. These blocks involve using local anesthesia to numb specific areas of the body, and the study aims to see how different doses and volumes of this anesthesia affect how quickly patients feel relief from pain and regain movement after the procedure. The goal is to complete these surgeries successfully while minimizing discomfort for patients.
To participate in this trial, you need to be between 18 and 65 years old, have a body mass index (BMI) between 18 and 35, and be scheduled for certain upper limb surgeries (not including shoulder surgery). Some individuals may not be eligible, including those who are pregnant, have severe breathing problems, or specific medical conditions. If you decide to join the trial, you can expect to receive the truncal block during your surgery, and the researchers will closely monitor your recovery and comfort levels afterward. Your participation could help improve pain management techniques for future patients undergoing similar surgeries.

Eligibility criteria
- Inclusion Criteria:
- • ASA 1-2
- • Body Mass Index (BMI) of 18-35 kg/m²
- • Age: 18-65 years -Patients undergoing all upper extremity orthopedic surgeries, except shoulder surgeries (distal humerus, elbow, forearm, hand, and wrist), will be included. -
- Exclusion Criteria:
- • Patient refusal
- • Pregnancy or breastfeeding
- • Severe pulmonary disease
- • Contralateral diaphragm paralysis
- • Nerve injury secondary to trauma
- • Neuromuscular disease
- • Peripheral neuropathy
- • Bleeding diathesis
- • Allergy to local anesthetics and dexamethasone
- • Uncontrolled diabetes mellitus
- • Morbid obesity with BMI \>35
- • Severe cardiovascular, renal, or hepatic disease
- • Infection at the site of the nerve block
