This clinical trial is studying how a video tool can help older adults who are frail make important decisions about their future medical care. As people age, they often face health challenges that make it hard for them to communicate their wishes if they become unable to do so later on. The study aims to create a video that will guide these individuals in understanding their options and help them express their preferences for care. By participating, older adults can learn more about advance care planning—essentially planning what kind of medical treatment they would want in the future—and feel more confident in making these decisions.
To be eligible for this trial, participants must be at least 65 years old, able to communicate, and diagnosed with frailty, which is a condition that can involve weakness and vulnerability. Those who have difficulty seeing or hearing the video, or who have already made their advance care decisions, cannot participate. Participants will receive the video and be invited to discuss their thoughts and feelings about their healthcare choices. The goal is to help them understand their options better and, hopefully, encourage them to officially document their healthcare wishes. This study is important because it aims to improve how healthcare teams can provide personalized care to older adults based on their individual preferences.

- Inclusion Criteria:
- • age \> or = 65 y/o can communicate and sign informed consent diagnosed to be frailty with clinical frailty scale 4-7
- Exclusion Criteria:
- • impaired visual acuity or hearing that can't watch video had signed advance decision before
