This clinical trial is looking to find out if taking creatine supplements, with or without a specific exercise program, can help improve physical ability and thinking skills in older adults aged 75 and up. The researchers want to know if creatine is safe for this age group and if it can really make a difference in their everyday lives. Participants will join a structured exercise program that includes strength training and balance activities to help with mobility and reduce the risk of falls. They will also be monitored for changes in their physical and cognitive abilities throughout the study.

To be eligible for this trial, participants must be 75 years or older and able to walk with or without assistance. They should have a life expectancy of at least six months and be able to give consent or have a legal representative do so. People with certain health issues that could make exercise unsafe or who have severe cognitive impairments will not be included. The study will involve about 120 participants, and it aims to provide valuable information on how physical activity and supplements can improve the quality of life for elderly patients.

Eligibility criteria

  • Inclusion Criteria:
  • Age: 75 years or older.
  • Able to ambulate with or without personal/technical assistance.
  • Life expectancy ≥ 6 months.
  • Barthel Index \> 60.
  • Informed consent by patients, or legal representatives.
  • Exclusion Criteria:
  • Global Deterioration Scale (GDS 6-7).
  • Intolerance or allergy foods or drugs.
  • Deglutition problems (Dx: dysphagia)
  • Any pathological or metabolic condition incompatible with physical exercise.
  • Creatine supplementation for less than 6 weeks.
  • Consumption of more than 200 mg of caffeine daily.
  • Supervised physical training.
