This clinical trial is studying a special program called the Nurse Education and Support Team (NEST) Program, which aims to help new mothers living in rural areas of North Carolina. The goal is to see if this program can provide better support for moms and their babies by offering more coordinated care for their physical, mental, and social health needs. Researchers want to find out if women in this program have fewer hospital visits after giving birth and follow health guidelines more closely, compared to those receiving standard care.
To participate in this trial, women must be at least 18 years old, have given birth at The Birth Center at Atrium Health Wake Forest Baptist, and live in one of the following counties: Davidson, Davie, Stokes, Wilkes, or Yadkin in North Carolina. Participants can expect to receive extra education and support during their postpartum period, which is the time following childbirth. This trial is currently recruiting women who meet these criteria and aims to improve health care for mothers and their infants in rural communities.

- Inclusion Criteria:
- • 1. Women that deliver an infant(s) at The Birth Center at Atrium Health Wake Forest Baptist and currently reside in Davidson, Davie, Stokes, Wilkes or Yadkin County in North Carolina
- • 2. Women that delivered an infant(s) at home, were transported to Atrium Health Wake Forest Baptist and received postpartum care at The Birth Center and currently reside in Davidson, Davie, Stokes, Wilkes or Yadkin County in North Carolina
- • 3. Ages 18 years or older
- Exclusion Criteria:
- • 1. Women that delivered an infant(s) and did not receive postpartum care at The Birth Center at Atrium Health Wake Forest Baptist.
- • 2. Women that currently reside outside of Davidson, Davie, Stokes, Wilkes or Yadkin County in North Carolina
- • 3. Under 18 years of age
