This clinical trial is studying a new way to help people with type 2 diabetes manage their condition using a mobile app called EMPOWER+. The trial focuses on community-based health coaching, which means that instead of just visiting a doctor occasionally, patients will receive ongoing, personalized support from health coaches in their daily lives. These coaches will help participants set achievable goals, create action plans, and stay motivated, which can lead to better management of their diabetes through improved medication adherence, healthier eating, and increased physical activity.
To join the trial, participants need to be at least 21 years old, diagnosed with type 2 diabetes, and have a recent blood test result indicating their diabetes is not well controlled (an HbA1c level of 6.5% or higher). They should also be able to read and write in English and live within a specific region in Singapore. Throughout the trial, participants can expect to use the EMPOWER+ app, which offers goal-setting features, educational resources, and personalized reminders based on their input. This study aims to see how effective and sustainable this approach is outside of traditional clinical settings, paving the way for broader use in the future.

Eligibility criteria
- Inclusion Criteria:
- • Aged 21 years and above
- • Having been diagnosed with type 2 diabetes mellitus
- • Having most recent HbA1c ≥ 6.5%
- • Physically able to exercise
- • Able to read, write, and converse in English
- • Able to comply with study procedures
- Exclusion Criteria:
- • Requiring assistance with basic activities of daily living (BADL)
- • Have planned major operation or surgical procedure within 6 months from the time of recruitment
- • Current pregnant or lactating
