This clinical trial is looking to find out if having a hospital pharmacist help dialysis patients can improve how well they understand and manage their medications. The study will first check how much the patients know about their treatment. It will compare two groups: one group will receive help from a pharmacist, while the other group will not. The researchers want to see if this support makes a difference in how patients understand their medications and if it helps improve their health.
To participate in the trial, individuals need to be over 18 years old and have been on dialysis (either peritoneal or hemodialysis) for at least three months. They should also be taking at least five daily medications and have not received help from a hospital pharmacist in the past for their kidney condition. Unfortunately, those who don’t speak or understand French, have certain cognitive challenges, or are not able to manage their own medications won't be eligible. Participants can expect to have their health monitored and to engage in discussions about their medication management with a pharmacist, which may help them feel more confident in handling their treatment.

Eligibility criteria
- Inclusion Criteria:
- • Patient over 18 years of age
- • On dialysis (peritoneal dialysis, hemodialysis) for at least 3 months
- • Prescription for at least 5 daily medications
- • Patient who has not received any pharmaceutical consultation from a hospital pharmacist as part of his or her treatment for chronic renal failure.
- Exclusion Criteria:
- • Patient does not speak and/or understand French
- • Patient's cognitive state does not permit an interview
- • Patient not autonomous in managing his or her daily medication (e.g.: patient in nursing home)
- • Patient refusal
