This clinical trial is investigating how well a specific pain management technique, called the erector spinae plane block, works in reducing the occurrence of lung problems, known as atelectasis, after bariatric surgery. The study aims to see if using this method is better than traditional pain relief methods in helping patients recover after surgery. Bariatric surgery is a procedure intended to help people with obesity lose weight.
To participate in this trial, patients need to be between the ages of 65 and 74 and have a body mass index (BMI) of 35 or higher, indicating obesity. However, those with certain health issues, like severe heart or lung problems, or conditions that affect their ability to understand or cooperate, cannot join. Participants will receive either the erector spinae plane block or standard pain relief after their surgery and will be monitored for any lung complications. This trial is important because it could help improve recovery and comfort for patients undergoing bariatric surgery.

Eligibility criteria
- Inclusion Criteria:
- • patients with body mass index ≥35 kg/m2
- • scheduled to undergo laparoscopic bariatric surgery.
- Exclusion Criteria:
- • American Society of Anesthesiologists (ASA) physical class\> III.
- • Subjects with contraindications to peripheral regional anesthesia blocks
- • neurological or psychological disorders
- • inability to cooperate
- • pre-existing chronic pain or cognitive dysfunction
