This clinical trial is focused on understanding how farmers and fishermen manage their health when they have chronic medical conditions like high blood pressure, heart disease, high cholesterol, or type 2 diabetes. The researchers want to find out if these individuals are aware of their conditions, how well they are being treated, and how effectively they can take care of themselves. To do this, they will develop easy-to-use educational materials, including fun tabletop games, to help improve self-care practices among this group. The goal is to create resources that are not only helpful but also engaging, making it easier for participants to learn how to manage their health.
To be eligible for the study, participants must be farmers or fishermen aged between 18 and 75 who have been diagnosed with one or more of the mentioned conditions. They should be able to manage their daily activities and communicate in Taiwanese or Mandarin. However, individuals with certain serious health issues, such as a history of stroke or severe heart problems, will not be included. Participants will have the chance to contribute to valuable research that aims to improve health education and self-care practices in their community.

Eligibility criteria
- Inclusion Criteria:
- • Farmers and fishermen who have been diagnosed with CMDs
- • Aged between 18 and 75 years old
- • Able to take care of their own daily life and can walk to the unit or community activity center that cooperates with this project
- • Can communicate with Taiwanese and Mandarin, and agree to participate in the second and third years of this study
- Exclusion Criteria:
- • Those who have no household registration or live in an unstable place in the study area
- • Those who have had a stroke or myocardial infarction
- • Those who have kidney and heart failure diseases and have limited water intake
- • Those who have cognitive or mental disorders
