This clinical trial is studying the effects of two different ways of giving methadone—an opioid pain medication—before and during heart surgery. The researchers want to find out if taking methadone by mouth before surgery helps manage pain after surgery better than receiving it through an IV (a tube inserted into a vein) just before anesthesia starts. This study is important because it may help improve pain management for patients undergoing elective cardiac surgery.
To be eligible for this trial, participants need to be between 65 and 74 years old and scheduled for elective heart surgery as the first surgery of the day. However, people with certain health conditions, such as chronic pain requiring ongoing opioid medications, serious heart issues, or severe kidney problems, cannot take part in the study. If you join this trial, you will receive either oral or IV methadone, and the effects on your pain after the surgery will be closely monitored. This study is currently recruiting participants, and it aims to help doctors find the best way to manage pain for heart surgery patients.

Eligibility criteria
- Inclusion Criteria:
- • • Undergoing elective cardiac surgery as the first case of the day in an operating room
- Exclusion Criteria:
- • Chronic pain requiring opioid medications as an outpatient
- • Opioid use disorder on medication assistance treatment
- • Prolonged QTc \> 500ms
- • Chronic kidney disease with eGFR \< 30mL/min
- • Documented cirrhosis
- • Intolerance to methadone
- • Subsequent surgeries after index surgery
