This clinical trial is studying a new surgical method for fixing a ruptured Achilles tendon, which is a common injury that can occur in athletes or active individuals. The researchers want to see if a new two-layer surgical technique, followed by a structured rehabilitation program, can help patients recover better than the standard non-surgical treatment, also followed by rehabilitation. Participants in the study will be randomly assigned to either the new surgery group or the standard treatment group, and the outcomes will be monitored for one year to see which approach leads to better recovery in terms of pain, function, and overall quality of life.

To be eligible for the trial, participants must be diagnosed with a complete rupture of the Achilles tendon and should be seen by a doctor within 14 days of the injury. They must also be able to understand and read Danish and should not have certain health issues like diabetes, other serious injuries, or be on specific medications that could affect their recovery. If you join the study, you can expect to undergo either the new surgical technique or the standard treatment and then participate in rehabilitation to help regain strength and function in your ankle. This trial aims to improve recovery outcomes for people with this type of injury.

Eligibility criteria

  • Inclusion Criteria:
  • Diagnosed with a complete mid-substance Achilles tendon rupture based on clinical exam, including Thompson/Matles test, by an experienced physician.
  • Presented within 14 days from injury.
  • Understands and reads Danish.
  • No contraindications for MRI.
  • Exclusion Criteria:
  • Smoking
  • Diabetes
  • Other injuries affecting their lower limb function.
  • Contralateral Achilles tendon rupture.
  • Re-rupture.
  • Anticoagulation treatment.
  • Inability to follow rehabilitation or complete follow-up tests.
  • Immunosuppressive treatment, including systemic corticosteroid treatment.
  • Pregnant
