The Critical Care Recovery Program is a clinical trial designed to help patients who have recently left the Intensive Care Unit (ICU). Many ICU survivors face ongoing health challenges, including issues caused by medications they received during their hospital stay. This program aims to improve their health by setting up a special clinic where patients can receive follow-up care after leaving the ICU. The clinic will focus on reviewing and adjusting their medications, safely stopping any that are no longer needed, and monitoring for new health concerns. By doing this, we hope to reduce the chances of patients experiencing harmful side effects from medications and needing to return to the hospital.
To participate in this trial, patients must have spent a significant amount of time in the ICU or had serious health issues like severe trauma or unexpected complications. Eligible patients will have follow-up visits at the clinic, where they will work with a healthcare team to create a personalized health plan. The goal is to help ICU survivors live healthier lives and reduce the need for extra hospital visits. This trial not only aims to enhance individual patient care but also hopes to establish better post-ICU support across British Columbia in the future.

Eligibility criteria
- Inclusion Criteria:
- • Patient being discharged to ward, home or repatriated to local health centre AND ONE OR MORE OF
- • Duration of mechanical ventilation \>72 hours
- • Length of stay in critical care \>96 hours
- • Out of hospital arrest
- • Maternal critical illness
- • Trauma
- • Significant ICU delirium
- • Unexpected adverse outcomes of planned treatment
- Exclusion Criteria:
- • Patient discharged for palliative care or expected to die within the next two months
