This clinical trial is looking at two different methods of performing total knee replacement surgery for people with severe knee osteoarthritis, which is a common and painful condition that affects movement. The trial aims to compare "restricted" and "unrestricted" kinematic alignment techniques during surgery to see which one leads to better outcomes for patients. This is important because knee replacements are becoming more common, especially in younger individuals, and finding the best surgical approach can help improve recovery and overall quality of life.
To participate in this study, you need to have severe knee osteoarthritis that affects at least one part of your knee joint and may have a specific type of knee alignment called varus alignment (where the knees bow outward). However, if you have certain conditions—like knee joint infections, severe heart or lung problems, or specific kinds of knee malalignment—you won't be eligible. Participants will be monitored closely throughout the trial to evaluate how well the surgery works and how it affects their daily activities. This research is a step towards improving knee surgeries and helping many people live more comfortably.

Eligibility criteria
- Inclusion Criteria:
- • -Patients suffered from end stage knee osteoarthritis of grade four according to Kellgren-Lawrence classification in at least one of the three knee compartments, who have an osteoarthritic knees of varus Coronal Plane Alignment of the Knee classification
- Exclusion Criteria:
- • 1. Any valgus malalignment of the knee joint.
- • 2. Malalignment more than 10° or less than 3° varus of knee joint.
- • 3. Ligamentous laxity of the affected knee (medial or lateral collateral ligaments)
- • 4. any tumors or secondary neoplasia diseases
- • 5. Knee joint infection
- • 6. severe cardiopulmonary dysfunction.
