The OPTIONS Research Study is looking at a new program designed to help older adults recover better after they break a hip or leg due to osteoporosis, a condition that weakens bones and makes them more likely to fracture. The study aims to see if providing a specific plan that includes exercise, a healthy diet, and medications to strengthen bones can improve recovery and overall quality of life for older adults in skilled nursing facilities (SNFs) and after they return home. Researchers will be working with 32 SNFs across the U.S. and will compare the new OPTIONS program with the usual care that patients typically receive after a fracture.
To be eligible for the study, participants must be 65 years or older, admitted to a participating SNF, and expected to return to their homes. Unfortunately, those who do not speak English or Spanish, lack a working phone, are in hospice care, or have severe cognitive impairment cannot participate. If chosen for the study, participants will receive support and information to help them follow the best practices for recovery. The study will measure how well these programs work by asking patients about their physical abilities, quality of life, and any new falls or fractures they experience. This research could lead to better care for older adults recovering from fractures in the future.

Eligibility criteria
- Inclusion Criteria:
- • Aged 65 and over
- • Admitted to a participating SNF
- • Expected to be discharged back to the community
- Exclusion Criteria:
- • Not English or Spanish-speaking
- • Do not have access to a working phone
- • In hospice care or terminal condition (not expected to live at least 4 months)
- • Severe cognitive impairment (Minimum Data Set 3.0 Cognitive Function Scale (MDS-COGS) score \> 5: 0, 1 represents cognitively intact-mild impairment, 2, 3, 4 represents mild-moderate impairment, 5, 6, 7, 8 represents moderate-severe impairment)
