This clinical trial is looking at ways to reduce shoulder pain that some people experience after having robot-assisted surgery to repair a hiatal hernia, which is a type of hernia that occurs when part of the stomach pushes through the diaphragm. The researchers want to see if using lower pressure to fill the abdomen with carbon dioxide during the surgery can help lessen this pain. Participants will receive standard care for their surgery, but they will also keep track of their pain levels afterward to help the researchers understand the results better.

To take part in this trial, you need to be between 65 and 74 years old and planning to have elective robot-assisted hiatal hernia repair surgery. Unfortunately, if you are under 18, have certain medical conditions, or if you are pregnant, you won't be eligible. If you join the trial, you will have regular follow-up appointments after the surgery just like everyone else, and your experience will help improve care for future patients.

Eligibility criteria

  • Inclusion Criteria:
  • patients undergoing elective robot assisted hiatal hernia repair
  • Exclusion Criteria:
  • patients less than 18 years of age
  • conversion to open surgery
  • BMI \> 40
  • history of abdominoplasty
  • history of chronic pain and/or opioid dependence
  • history of COPD and/or supplemental oxygen use
  • pregnant patients
  • incarcerated patients
  • patient refusal
