This clinical trial is looking at two different methods of giving spinal anesthesia to elderly patients who are having surgery for hip fractures. Spinal anesthesia is a way to numb the lower half of the body, and this study aims to find out if using ultrasound to guide the anesthesia (the "modern" method) is better and safer than the traditional method that relies on the doctor feeling for landmarks on the body (the "blind" method).
To participate, patients need to be over 65 years old and scheduled for hip fracture surgery. They should also be in generally good health, as determined by a standard classification used by anesthesiologists. Patients who have certain health issues or previous back surgeries may not be eligible. Those who join the study will be randomly assigned to one of the two methods of anesthesia. Throughout the trial, participants can expect to receive care and monitoring just like they would in a regular surgery setting, with the goal of improving safety and effectiveness for future patients.

Eligibility criteria
- Inclusion Criteria:
- • all consented patients aged over 65 years old scheduled to undergo surgery for hip fracture under continuous spinal anesthesia with American Society of Anesthesiologists ASA physical status classification from I to III
- Exclusion Criteria:
- • contraindications to spinal anesthesia ( allergy to local anesthetic, coagulopathy, infection at the site of injection)
- • previous history of lumbar spine surgery
- • per operative severe complication with conversion to general anesthesia
