This clinical trial, called "Culturally Engaged REcovery: MOms Connected Through Native CommunitY," is focused on helping pregnant and new mothers from American Indian and Alaska Native communities who are dealing with substance use disorders. These mothers face a significantly higher risk of serious health issues and complications during and after pregnancy compared to other groups. The goal of the study is to adapt existing care models to better meet the specific needs of these mothers, combining prenatal and postpartum care with support for substance use treatment. The trial will take place at Sacred Circle Healthcare in Salt Lake City, Utah.
To participate in this study, women need to be pregnant, speak either English or Spanish, and identify as Indigenous, Native, American Indian, or Alaska Native. They should plan to carry their pregnancy to term and be willing to allow the study team to gather some medical information from other healthcare providers. Participants will not be accepted if they have had a serious mental health episode recently or if they are close to their due date. While the trial is not yet recruiting participants, those who join can expect to receive personalized care tailored to their cultural background and specific health needs. This study aims to improve the health and well-being of Native mothers and their babies by addressing the challenges they face during pregnancy.

Eligibility criteria
- CEREMONY Inclusion Criteria:
- • English or Spanish speaking
- • pregnant (verified by point of care urine pregnancy test)
- • plan to carry the fetus to delivery
- • identify as Indigenous, Native, Native American, American Indian or Alaska Native
- • willing to grant a release of information to allow study staff to contact other health care institutions and treatment centers and collect information from the medical record, e.g., date of delivery, infant admission etc.
- CEREMONY Exclusion Criteria:
- • have a documented psychotic episode in the last 30 days
- • be \>39 weeks of gestation
- • be unable provide collateral contact information of at least 1 person
- • be unable to provide reliable phone number
- • plan to move within 3 months of delivery
