This clinical trial is investigating how low levels of phosphate in the blood, known as hypophosphatemia, affect the ability of patients to breathe without a ventilator after being on mechanical ventilation. Researchers believe that low phosphate levels may weaken the respiratory muscles and hinder the nerves that help control them. By correcting these low phosphate levels with intravenous supplements, the study aims to see if patients can breathe more effectively and potentially leave the ventilator sooner.
To participate in this trial, individuals must be at least 18 years old, have been on a ventilator for more than 48 hours due to acute respiratory failure, and be in a stable condition ready to wean off the ventilator. Participants will have a special device placed to monitor their diaphragm activity and will undergo a series of assessments related to their breathing and phosphate levels. The study is currently recruiting participants and aims to improve the understanding of how phosphate levels impact recovery from mechanical ventilation.

Eligibility criteria
- Inclusion criteria:
- • age ≥18 years old; Acute respiratory failure, tracheal intubation patients; Invasive ventilator use time ≥48 hours; Clinically determined remission of primary disease, has switched to auxiliary ventilation mode and plans to go offline in the near future.
- Exclusion criteria:
- • patients with severe neuromuscular disease; Patients with central system diseases; Patients using muscle relaxants for more than 48 hours; Patients who have been or are about to be discontinued from life support; Patients with contraindications for gastric tube insertion after esophagogastric fundus varices, digestive tract perforation, and upper digestive tract surgery; Pregnant patients; Ultrasonic window is poor.
