The IAMABLE trial is studying a new web-based app designed to help people with chronic conditions, such as neurological, musculoskeletal, or cardiovascular diseases, manage their rehabilitation at home. The app, called IAMABLE (I am able), shares evidence-based strategies to support recovery and improve daily life. In this study, 50 participants aged 45 to 75 who live in the community and have at least one of these chronic conditions will be recruited. To participate, individuals must have access to a computer or mobile device, be able to understand instructions in English, and score well on a cognitive assessment.
Participants in the trial will use the IAMABLE app for four months, while a comparison group will have access to general health information online. Researchers will track various aspects of participants' health, including their quality of life, mobility, and ability to manage their condition, over the course of the study. This trial will help determine how effective the app is and how well it encourages users to engage with their rehabilitation. Ultimately, the goal is to create a valuable resource for people with chronic illnesses to support their journey toward better health and independence.

Eligibility criteria
- Inclusion Criteria:
- • is community-dwelling, has access to a computer or mobile device, has an email address, is able to follow directions in English, self-reports at least one of three types of prevalent chronic conditions associated with disability that frequently require rehabilitation supports (i.e., musculoskeletal, cardiovascular, and neurological conditions), responds "somewhat easily" or "very easily" to questions on the Modified Computer Proficiency Questionnaire (MCPQ-12).
- Exclusion Criteria:
- • scores \< 11/15 on the Montreal Cognitive Assessment (MoCA) 5-Minute Test (Version 2.1).
